: My last working day with my previous employer, Blue Sabre, Inc., was on 1/25/02 due to the companies lack of incoming orders. So I was one of many
persons who were laid off.
: Prior to this last day I had inquired of the office administrator/co-owner as to the status of our group medical insurance plan and with regards to effects
and coverage duration.
: The administrator's reply was on that I was to be covered under my existing plan until termination on 4/25/02.
: On 2/10/02 as required by the provisions to comply with my filing for unemployment benefits, my local EDD office, Glendora, CA stated I must attend a
workshop and orientation.
: During this workshop I heard the phrase "COBRA" and asked it's meaning.
: The State EDD case worker informed me that my previous employer was required to either hand to me or mail to me the information regarding my
eligibility and the %'s that I contributed and that of which my employer contributed, along with the coinsiding equal dollar amounts of each. Also to whom I
was supposed to contact and by what means.
: Thus far I contacted the administrator by phone on 2/12/02 and again was told it was not her responsibilty to do so. I followed up the call again requesting
the information and was told that the insurance carrier would provide me the information within 30 days after she informed them to terminate me.
: I am under a continual maintained medical status and require medication on a daily basis and will have to for the duration of my life time. I'm a Bi-Polar
Manic Depressive and currently require two different medications to remain stable.
: I have exhausted all avenues and was informed of your being a possible source of help.
