Judging from your previous 3/13 posting on this forum, it would seem you must have already given "notice of loss" to your insurance company. However, if you haven't yet notified your insurance company of your loss, you may want to do so immediately. A completed Statement of Loss form is an informal amount of loss submission. A completed Proof of Loss form is a formal claim submission; although many policies no longer allow for Proof of Loss filing and have abolished the word "claim." Both forms should be available through your insurance company, and are also included in the book "Policy Ensurance." Claim filing and form information can be found at Deserve
